Washington must stop using state funds for sex-change procedures on minors

Family Brief Staff

The cutoff took effect Oct. 13, but Washington is using state funds to keep Apple Health coverage for minors and suing to challenge the federal rule.

Washington should end taxpayer funding for minors’ sex-change procedures, not shift the bill from Medicaid to state-only funds: “Washington state will no longer cover these services under Medicaid funding, but will continue to cover these services using state-only (non-Medicaid) funds.”1 On Oct. 27, 2026, @DIAGWashington reported the Commission emailIn its Oct. 27, 2026, post, @DIAGWashington reported the Commission email: “The WA State LGBTQ Commission just sent an email announcing that the state's Apple Health program (Medicaid) will now cover the cost of "gender affirming" drugs and surgeries - FOR MINORS UNDER 18.2 @DIAGWashington’s post also said, “The Federal Medicaid program stopped funding these on Oct. 13th.” The HCA notice says CMS issued its final rule on Aug. 13, 2026, and lists Oct. 13, 2026, as the amendments’ effective date. The Health Care Authority’s Transhealth program says, “State funds are used to maintain coverage of medically necessary gender‐affirming care for Apple Health clients age 18 and younger through fee-for-service.”

The HCA Transhealth program lists “Puberty suppression therapy,” “Hormone therapy,” and “Hospitalizations and physician services related to procedures performed for gender-affirming surgery.” HCA’s surgery requirements include having “Been on gender affirming hormone therapy for a minimum of 12 months preceding surgery (with the exception of mastectomy/reduction mammoplasty).”3 Attorney General Nick Brown said: “The federal administration is once again claiming powers it does not have—this time, in order to foreclose healthcare options that should be left to Washington youth, their doctors, and their families.” His office estimated that “the state of Washington would have to pay about $1 million annually to cover the portion of Medicaid and CHIP costs previously covered by the federal government. ”4

State funds keep blockers and hormones covered

The program lists hormone therapy and puberty suppression therapy among covered services. Its surgical coverage includes related hospitalizations and physician services3.

A surgery-only objection would leave the drug coverage untouched. A funding restriction should reach the drugs before a child ever reaches an operating room.

Apple Health covers treatment, not reversal

The same HCA page states: “Apple Health does not pay for procedures and surgeries related to reversal of gender-affirming surgery.”3

For parents weighing what the state offers their child, that boundary deserves a direct answer. Why should public money support a surgical transition but stop at procedures related to reversing that decision? Officials should have to defend the exclusion alongside the coverage they promote. Families should not mistake a promise to pay for an intervention for a commitment to pay for reversing it.

Washington sued to restore federal funding

Washington Attorney General Nick Brown’s office announced Sept. 2 that Washington, 20 other states and the District of Columbia had sued to challenge the federal rule4.

Parental authority does not require taxpayers to finance sex-change procedures on children.

Parents should demand an end to state funding

Washington’s governor and lawmakers should end state funding for minors’ puberty blockers, cross-sex hormones and sex-change surgeries. Brown should stop seeking to restore the federal subsidy. Children and families deserve support without the state underwriting medical transition.

Parents should put the spending choice directly to their representatives. An answer about federal compliance cannot excuse an independent decision to spend state money. Nor does invoking doctors’ discretion settle what taxpayers should buy. Will you end Washington tax funding for sex-change procedures on minors3—yes or no?
